Frequently Asked Questions
General
Elevate Mind Performance is a fee-for-service practice and is not currently in-network with insurance plans. Payment is collected directly through the secure patient portal.
We can provide documentation, such as a superbill, that clients may submit to their insurance company for possible out-of-network reimbursement. Reimbursement is not guaranteed and depends on each individual insurance plan.
We work with adolescents, teenagers, and adults seeking support with mental health, emotional resilience, stress, anxiety, depression, focus, performance, life transitions, and overall well-being.
While the practice has a special focus on athletes, students, professionals, and high performers, care is not limited to those groups.
No. Athletes are one important focus, but Elevate Mind Performance also works with students, professionals, adolescents, teenagers, and adults from many backgrounds.
The practice integrates mental health care, performance-focused strategies, psychotherapy, lifestyle medicine, and thoughtful medication management when appropriate.
No. Elevate Mind Performance does not currently have a physical office location. Services are provided through secure telehealth.
Clients must be physically located in a state where the clinician is legally authorized to provide care at the time of the appointment.
Yes. Every new client is asked to complete a 15-minute complimentary consultation before scheduling a clinical or performance service.
This call helps determine whether the practice is the right fit and guides you toward the most appropriate next step
Most clients begin with a Comprehensive Initial Evaluation, a 75-minute appointment focused on understanding your history, current concerns, goals, prior treatment, medical history, medications, lifestyle factors, and relevant records.
When appropriate, standardized questionnaires or screening tools may be used to better understand symptoms, functioning, and progress over time.
Clients are encouraged to upload or bring any pertinent medical or mental health records before the appointment.
This may include medication lists, prior evaluations, lab results, psychological or neuropsychological testing, hospital records, school accommodation documents, or records from other clinicians.
Yes. We evaluate concerns related to ADHD, attention, concentration, executive functioning, and performance.
Because attention difficulties can have many causes, an ADHD evaluation may require multiple appointments, rating scales, collateral information, review of records, and/or additional testing to ensure the diagnosis is accurate and treatment recommendations are appropriate.
When clinically appropriate, we may recommend or provide orders for labs or other medical studies to support diagnostic clarification and treatment planning.
However, Elevate Mind Performance does not perform labs, imaging, or medical procedures directly. These studies are completed through outside laboratories, primary care offices, specialists, or imaging centers.
Telehealth is effective and convenient for many concerns, but it may not be appropriate for every clinical situation.
High-complexity cases, acute safety concerns, severe substance use, active psychosis, significant medical instability, eating disorders requiring intensive monitoring, or situations requiring frequent in-person care may require referral to a higher level of care, in-person services, emergency services, or a more specialized treatment setting.
Elevate Mind Performance does not provide emergency or crisis services. If you are experiencing a life-threatening emergency, suicidal thoughts with intent, risk of harming yourself or others, or a medical emergency, call 911 or go to the nearest emergency department.
Your privacy is very important. Information shared in treatment is kept confidential in accordance with applicable privacy laws and professional standards, with limited exceptions such as safety concerns, suspected abuse or neglect, court orders, or other situations required by law.
